Based on the assessment, the Care Manager will explore care options to address any identified gaps or deficiencies.

A home tour will identify potential safety concerns, such as trip hazards, lack of grab bars, or improper use of space heaters or extension cords.

The Care Manager will evaluate risks related to isolation and loneliness, considering physical and emotional health.

A review of health concerns, medication management, and other issues will shape a personalized care coordination plan.
